We are excited to see you and your pet! Our team will respond to your request as soon as possible and help book your appointment. For URGENT requests please call 919-219-4919. Because of the nature of medicine and travel we schedule appointments in morning and afternoon arrival windows; you will be able to list your preference on the form.

A deposit is required for all appointment reservations (typically the travel fee & exam for one pet). The reception team will collect payment when the appointment details are confirmed. This form does NOT set up an appointment or request payment, it sends the request to our team.

THE RECEPTION TEAM WILL REACH OUT TO FINALIZE YOUR APPOINTMENT AND COLLECT YOUR APPOINTMENT RESERVATION DEPOSIT, YOU DO NOT HAVE A SCHEDULED APPOINTMENT AT THIS TIME.

*Please be aware that if we are unable to fully examine your pet due to behavior concerns, we still charge the full house call fee. We will make all reasonable and safe efforts to examine your pet.

*Please know that we cannot provide emergency care, hospitalization, full sedation, x-rays, or surgery. We encourage you to have a relationship with a brick and mortar hospital for radiology, surgeries, and day-time emergencies.

*Payment is expected at time of service. We accept cash and all major credit cards.

*If you cancel your appointment within 48 business hours, you forfeit your reservation deposit.
