New Client/Patient Form


(Please submit a separate form for each Patient)

We require all new clients and existing clients with new pets to fill out this form. If you are a new client, here are a few key points to know up front:

Appointment Reservation Deposit required to confirm an appointment (typically the travel fee & exam for one pet) This money will be a credit on your account and will go towards your appointment.
● Cats need to be confined to a bathroom or similar so that they can’t hide under furniture.
● Dogs need to wear a collar and leash during the visit.
● Appointment arrival times are 9-noon or noon to 3 (you can provide additional details regarding your availability).
● We will reply to you as soon as we possibly can. We are not an emergency service, but if you have an URGENT need, please call 919-219-4919.

BEHAVIOR INFORMATION / REQUIREMENTS


We want to make our visit as stress free for you and your pet as possible. By creating a low-anxiety, positive experience for our patients, we can help decrease the fear, anxiety, and stress of veterinary visits over time. We do not blame pets for being scared of the veterinarian as we often reinforce their fear by doing things that they do not experience in their typical day-to-day-lives (restraining them, touching parts of their body they are not used to, and often performing mildly uncomfortable procedures like blood sampling or vaccination). You can really help your pets out by practicing opening their mouths and touching their ears and feet. Practice restraining your animal and counting to 30 so they learn to be still for a period of time. Have friends/strangers greet, pet, and give them treats!

If you or our staff determine the appointment can not proceed as is due to temperament, we will then discuss using a muzzle or E-collar, and even possibly returning for a recheck visit where your pet is on anti-anxiety medications so that we can improve the comfort and safety for you, your pet and our staff at all visits. By decreasing their anxiety we attempt to counter-condition them to accept veterinary visits as they learn it is not as scary as they once thought.

Please be aware that if we are unable to fully examine your pet due to behavior concerns, we still charge the full house call fee. We will make all reasonable and safe efforts to examine your pet. We want this to get better over time and not make the animal more fearful in the long term. We love animals and we want them to love us too!

*Please know that we cannot provide emergency care, hospitalization, full sedation, X-rays, or surgery. We encourage you to have a relationship with a brick and mortar hospital for radiology, surgeries, and day-time emergencies.

*Payment is expected at time of service. We accept cash and all major credit cards.

*If you cancel your appointment within 48 business hours, you forfeit your reservation deposit.
